First Case in Hengshui! Hengshui Tongrui Eye Hospital of Huaxia Eye Group Performed FCB Surgery, Offering a New Treatment for Retinal Detachment Patients

On July 11, Director Xi Jiansong of Hengshui Tongrui Eye Hospital of Huaxia Eye Group successfully performed the first foldable capsular buckle(FCB) implantation in Hengshui. The FCB was successfully implanted for a patient with rhegmatogenous retinal detachment, enabling the reattachment of the patient's detached retina. As the latest new surgical method for the treatment of rhegmatogenous retinal detachment, FCB implantation is a special type of scleral buckling surgery with minimally invasive characteristics and fewer complications. After the operation, patients can take a free position, which greatly improves the overall surgical experience and provides a safer and more efficient surgical plan for patients.

 

Condition Overview

The patient is a 20-year-old female from Hengshui. Two weeks ago, she experienced distorted vision and blurred eyesight in her right eye without obvious inducement. She had visited a nearby hospital but did not undergo detailed examinations, and was advised to seek treatment at a higher-level hospital. The patient then turned to Hengshui Tongrui Eye Hospital of Huaxia Eye Group for help. She was admitted to the hospital with a diagnosis of right eye rhegmatogenous retinal detachment.

 

Director Xi Jiansong's team examined her eyesight: right eye 0.5, left eye 1.0. Both eyes had intraocular lenses in place. The right eye had a retinal detachment ranging from 9:30 to 2:00, involving the macular area, and a round hole of about 1/5 PD in size was visible at the equator at 10:30.

01 Introcular Surgery vs. Exraocular Surgery
Introcular surgery, namely vitrectomy (PPV) surgery, has great and irreversible damage, slow recovery, high cost, and requires prone position for two to three weeks after surgery. If silicone oil is filled, another operation is needed to remove it after one to three months. Vitrectomy can destroy the oxygen gradient of the vitreous, and irreversible conditions such as cataract and optic nerve damage are likely to occur.
More doctors recommend external surgery, which does not need to enter the eye, thus ensuring that the oxygen balance of the eye itself will not be destroyed. The traditional external surgery is scleral buckling, which has less damage than endoluminal surgery. Once successful, there is no need for repeated operations, reducing the patient's pain.
 
02 Foldable Capsular Buckle (FCB) Surgery
Foldable capsular buckle (FCB) retinal reattachment surgery is a type of extraocular surgery. Compared with traditional extraocular retinal surgery, FCB has the advantages of shorter operation time (reduced from 50 minutes to 10 minutes), no retrobulbar anesthesia, no muscle traction, no scleral fluid release, no intraoperative positioning, and no hole freezing. Doctors only need to implant the folded uckle into the outer wall of the eye at the detachment site, and then inject saline to inflate the capsule, which will "push" the detached retina back like a jack. The postoperative recovery is fast, which greatly reduces the patient's pain, avoids many postoperative complications, and can greatly improve the treatment experience of "retinal detachment" patients. The operation adopts three-dimensional reconstruction calculation to accurately judge the position of the hole, which can effectively improve the correctness and reattachment rate of the hole reattachment.
